Appointments

Our Surgical Practice is Referral-Based.      

 Appointments for consultation may be scheduled by your Family Physician or Allied Health Professional (i.e. Physical Therapist, etc.).  Prior to your appointment a packet of information will be forwarded to you containing demographic and medical history questionnaires.  Completion of these forms in their entirety prior to your appointment time will aid in avoiding untimely delays.  We also request that our new patients obtain medical records as well as recent tests and x-rays in preparation for the initial consultation.

Our Physicians strive to see you at your scheduled appointment time.  Unfortunately, there are times when emergencies arise and unavoidable delays occur.  Should we learn that one of our doctors is  unable to make it into the office due to an emergency situation, we will make every effort to contact you in time to reschedule your appointment.  In turn, should you be unable to keep your scheduled appointment, please contact our office at least 24 hours in advance to cancel and/or reschedule.
